With new regulations for online pharmacies coming into effect, many people are questioning whether they are eligible for weight loss injections. While these medications can be an effective tool for weight management, they are not suitable for everyone. Decisions around prescribing weight loss injections are based on medical guidelines, clinical judgement, and an individual’s overall health.
Our Lead Nurse, Stephanie Smith is here to share some insights into how clinicians assess whether someone is a suitable candidate for weight loss injections – and why medical supervision is crucial for safety and success.

Understanding Weight Loss Injections
Weight loss injections, including liraglutide (Saxenda), semaglutide (Wegovy), and tirzepatide (Mounjaro), work by mimicking natural hormones in the body to regulate appetite and blood sugar. They help individuals feel fuller for longer, reducing food intake. However, these medications are not a quick fix: they must be combined with a healthy diet and regular exercise to be effective.
Because of their impact on metabolism and potential side effects, weight loss injections should only be taken under medical supervision and in accordance with strict eligibility criteria.
Who is Eligible for Weight Loss Injections?
BMI and Health Conditions
The primary eligibility requirement for weight loss injections is body mass index (BMI). However, BMI alone is not enough. Clinicians also consider weight-related health conditions that may put individuals at higher risk of serious complications. Each medication has specific criteria under NHS guidelines:
Liraglutide (Saxenda)
- Suitable for adults up to 75 years old
- BMI of 35 or more, or 32.5 or more for individuals of Asian, Black, Middle Eastern, or African-Caribbean origin
- Must have non-diabetic hyperglycaemia (high blood sugar)
- Must be at high risk of heart problems, such as high blood pressure or cholesterol
Semaglutide (Wegovy)
- BMI of 35 or more, or 32.5 or more for individuals of Asian, Black, Middle Eastern, or African-Caribbean origin
- Can be prescribed at a BMI of 30–34.9 (or 27.5–32.4 for certain ethnicities) if additional health conditions apply
Tirzepatide (Mounjaro)
- BMI of 35 or more, or 32.5 or more for individuals of South Asian, Chinese, Middle Eastern, Black African, or African-Caribbean origin
- Must have at least one weight-related health problem, such as high blood pressure or type 2 diabetes

Medical History and Contraindications
Before prescribing weight loss injections, clinicians carefully review a patient’s medical history to ensure the medication is safe for them. Some possible contraindications are:
- Not suitable for those who are pregnant, breastfeeding, or planning pregnancy
- Not recommended for individuals with severe liver or kidney conditions
- Tirzepatide may impact contraceptive pill absorption – patients should use an additional contraceptive method for four weeks after each dose increase
Why Medical Supervision is Essential
As a prescribing nurse, I can’t emphasise enough the importance of clinical oversight when using weight loss injections.
Many people are eager to try weight loss medications, especially with the rise of social media trends. However, these treatments come with risks, and medical supervision is essential to ensure safety. Patients need ongoing monitoring to manage side effects, adjust dosages if needed, and prevent potential complications.
Potential side effects include nausea, vomiting, reflux, and digestive issues, which can be challenging to manage without professional guidance. Clinicians also assess the psychological impact of weight loss, ensuring that patients receive both physical and emotional support throughout their journey.

The Right Treatment for the Right Person
Weight loss injections can be life-changing for individuals who meet the criteria and use them responsibly. However, they are not a shortcut to weight loss and should only be considered when lifestyle interventions alone have not been effective.
Anyone considering weight loss injections should speak to a qualified healthcare professional to determine their suitability and ensure they are using the medication safely.
For those who do qualify, ongoing medical support is key to achieving the best possible outcomes – helping patients manage side effects, monitor progress, and develop sustainable habits for long-term weight management.
